Borang Aduan

Ini adalah borang yang perlu anda isi jika anda ingin mengemukakan aduan kepada Phoenix. Untuk bantuan dalam melengkapkan borang ini, sila hubungi salah seorang wakil sokongan pelanggan kami di [email protected].

Sila isi borang dan klik pada butang “Hantar Aduan Anda” di bawah untuk menyerahkan aduan anda dengan segera dan secara automatik kepada Phoenix LTD. Harap maklum bahawa Syarikat berhak untuk tidak menerima aduan yang dikemukakan melalui sebarang cara atau kaedah lain (cthnya. E-mel, telefon, dsb.).

Maklumat yang lengkap, terkini dan tepat mesti diberikan kepada Phoenix LTD untuk tujuan menyiasat dan menilai aduan anda. Sila ambil perhatian bahawa borang aduan di bawah adalah indikatif dan tidak lengkap. Phoenix LTD boleh meminta maklumat tambahan dan/atau penjelasan dan/atau bukti mengenai Aduan anda. Selain itu, Phoenix LTD boleh meminta anda menyerahkan semula borang aduan baru jika ia percaya bahawa anda telah melengkapkan dan/atau memalsukan borang aduan anda.

Phoenix LTD akan berusaha untuk menyelesaikan aduan anda atas dasar niat baik, ekuiti dan mengambil langkah yang konsisten dengan amalan pasaran.

I acknowledge that Phoenix LTD will give proper notification about my complaint within five (5) days from the receipt of my complaint and provide me with a unique reference number. The unique reference number should be used in all your future contact with the Company, the Seychelles Financial Services Authority (FSA) regarding the specific complaint. We shall make every effort to investigate your complaint and provide you with the outcome of our investigation within thirty (30) days from today. In case additional information is required, you shall be contacted by one of our officers. If the Complaint is more complex and takes longer than thirty (30) business days to resolve, we will communicate the reasons for the delay.
